Required Forms
 
As with all events, there is some paperwork involved with the process.  At our fair, we have BOTH school forms and international affiliation forms that MUST BE COMPLETED AND SUBMITTED in order for you to participate.  Please take time to READ THROUGH ALL forms listed below in the TWO sections (ISEF Forms and WCA Forms) -- EVERY student will have SOME of BOTH TYPES OF FORMS to submit according to the deadlines set by their teachers.*  
 
*Some teachers MAY have EARLIER deadlines than the ones set on the Fair's Master Schedule -- you are responsible for meeting YOUR TEACHER'S deadlines as he/she may set them.  The Master Schedule is to give you a SNAPSHOT into the overall schedule that the Fair MUST follow and which YOUR TEACHER is required to meet FOR your class!
 
ISEF Forms
Because we are an ISEF (International Science & Engineering Fair)-Affiliated Fair, there are some standard protocols to which we must adhere.  These protocols include the use of ALL ISEF-mandated paperwork from the inception of your project to your participation in the WCA Fair on January 18th (and beyond, where applicable!).  Although you may obtain any and all of these forms from your teacher in hardcopy, you may also download them by simply clicking on the link provided for each form preceding its description.

*Any extension on this deadline MUST be obtained from your science teacher PRIOR TO Thursday, August 20th.
 
These forms must be kept with your project AT ALL TIMES and MUST be included in your RESEARCH PAPER submittal to your teacher on Friday, December 5th AND at your project on the day of the Fair.
 
Those forms which MUST be obtained from EVERY student (individual and EACH team member!) regardless of their project topic are Forms 1, 1A and 1B.
 
Form 1: Checklist For Adult Sponsor Required for ALL students. For information as to the exact definition of an adult sponsor, please read HERE.
 
Form 1A: Student Checklist and Research Plan Required for ALL students.  This form also includes the definition of the Reseach Plan and what is required for such.
 
Form 1B: Approval Form Required for ALL students.
 
Other forms which MAY apply to your project should be investigated through your science teacher as ALL Forms are due on August 22nd -- read each of the descriptions below to see if any of these forms could apply to your project.  If you have any questions, PLEASE consult with your science teacher immediately or contact our Fair Director, Mrs. Yvette Kromann.
 
Form 1C: Regulated Research Institutional/Industrial Setting Form This form must be completed by the adult supervising the student research conducted in a regulated research institution, industrial setting or any work site other than home, school or field.
 
Form 2: Qualified Scientist Form  This form must be completed by a supervising qualified scientist for student research involving human subjects, vertebrate animals, potentially hazardous biological agents, and DEA-controlled substances.  For information as the exact definition of a Qualified Scientist, please read HERE.
 
Form 3: Risk Assessment Form This form is r equired for projects using hazardous chemicals, activities or devices. You must obtain SRC/IRB approval before you begin experimentation. For detailed information regarding what qualifies as "hazardous", please read HERE.
 
Form 4: Human Subjects Form This form is required for all research involving human subjects. You must obtain SRC/IRB approval before you begin experimentation AND a copy of your complete questionnaire and/or survey MUST BE SUBMITTED WITH THIS FORM. There is also a Sample Informed Consent Statement provided by ISEF.

If you find yourself unsure as to the assessment of whether or not your Human Subjects project will be approved, you can view the complete Risk Assessment Guide that will be utilized by the SRC/IRB to evaluate your project's viability.
 
Form 5: Vertebrate Animal Form (5A and 5B) These forms are required for any research involving non-human vertebrate animals.  You must obtain SRC/IRB approval before you begin experimentation. Form 5A is to be completed for research conducted at a Non-Regulated Research Site. Form 5B is to be completed for research conducted at a Regulated Research Institution.
 
Form 6A: Potentially Hazardous Biological Agents Risk Assessment Form This form is r equired for research involving microorganisms, rDNA, fresh/frozen tissue, blood and body fluids. You must obtain SRC/IRB approval before you begin experimentation.
 
Form 6B: Human and Vertebrate Animal Tissue Form This form is r equired for projects using fresh/frozen tissue, primary cell cultures, blood, blood products and body fluids. All projects utilizing the tissues listed here must also complete Form 6A. You must obtain SRC/IRB approval before you begin experimentation -- this approval may cause you a time delay of up to a week as special permission from higher authority must be obtained before your research can be approved.
 
Form 7: Continuation Projects FormThis form is required for projects that are a continuation in the same field of study as a previous project. This form must be accompanied by the previous year’s: abstract, Form 1A and Research Plan.
 
****ISEF Abstract Form This form is ONLY for WCA finalists who are advancing to NARSEF.  Please see the fair director if you have any questions.****
 
WCA Forms
The two WCA Forms are not due until mid-November and are required for EACH PROJECT -- these forms are PARAMOUNT in the planning of the actual day of the Fair and CANNOT be late.  The on-time submittal of these forms will count as a TEST GRADE for the student.  As with the ISEF forms, you may obtain each form from your teacher in hardcopy, you may also download them by simply clicking on the link provided for each form preceding its description.

WCA Fair Registration Form (pdf)Required for ALL students! This registration form is required for your project's inclusion in the day of the fair -- it assures that we have enough judges for the fair AND that you will be in the correct category with the proper space and/or electricity needs! (for MS Word Format, click here)
 
WCA Abstract Form (pdf) Required for ALL students!This is the OFFICIAL form on which you MUST submit your project abstract.  This form MUST be included in your Research Paper and it MUST BE DISPLAYED vertically** at your project on the day of the fair! **Note: we suggest the use of a simple picture frame or attaching it to the table edge in front of your project -- DO NOT include your abstract on your display board itself! (for MS Word Format, click here)
